Microphlebectomy Austin
Sometimes small, problematic veins are not accessible with EVLT or Sclerotherapy. In these cases, Dr.Hovorka may perform a Microphlebectomy or Phlebectomy at his Austin, Texas practice, All In Veins. This minimally invasive surgery can be conducted with local, regional, or general anesthesia depending on the extensive nature of the veins. Prior to treatment a venous ultrasound study is conducted. Based on the results a Microphlebectomy may be recommended. This procedure involves injecting local anesthesia around the varicosities. This technique uses incisions so small, generally no suture is necessary to close them. Compression will be applied for two weeks for healing and you will be able to return to normal activities immediately. Due to its simplicity, this technique can be used in many different cases with good aesthetic results.

How is the Microphlebectomy Procedure Performed?
First, photographs are taken of the veins in the leg to be treated. These photographs, with patient's consent, are used for insurance, research, and marketing purposes. The veins are then marked with a skin marker while the patient is standing so they can be visualized better. The leg is washed with a solution to kill any germs on the skin and to prevent infection. Sterile drapes are then applied and the skin over the veins is numbed with a local anesthetic. A different numbing medicine, with epinephrine in it is sometimes injected deeper around the vein to be removed to constrict the vein. Once the area is numb, a very small incision is made over the top of the vein and a small hook is used to located the vein and pull it up through the skin to be removed.
A pulling sensation is usually felt, but without pain. This process is repeated until all of the varicose veins that were marked are gone. The skin is then cleansed and a small adhesive strip is placed over the incisions to hold the skin edges together. An absorbent pad is placed over these areas to catch any drainage from the injected medicine and a dressing is placed on the areas treated for the next 24 hours.
What is the Down-time after Microphlebectomy?
Mild to moderate pain can be expected after Microphlebectomy as will as bruising. A patient should take the entire day off work and keep his/her leg elevated as much as possible. Usually dressings are worn for the first 24 hours then compression is worn while a patient is up and moving for about two weeks following this procedure.What Are the Risks of This Procedure?
Only mild complications have been associated with Microphlebectomy. These include: bruising, swelling, scar formation, permanent pigmentation of the skin, temporary phlebitis, superficial clot formation, minor infection, and mild allergic reaction to any medicine use. There is a remote possibility, as with any medical procedure, that more serious complications could occur including severe allergic reaction (anaphylaxis), deep venous blood clot, pulmonary embolism and significant bleeding.
